Answers: My advice would be to move about to autozone and buy a haynes repair manual i hold one for every car i own they contribute you a step by step complete tear down and start again of your vehicle. There around $17 and well worth yhe price.

Depends on the year of van. Some remove beside like star bits and remove the trim around it...try to find any screw around the fascia. More than likely you remove those screw and than the radio is mounted with a couple of screw that you take out to remove the radio.
