You don't want your reminder to do it online, you can do it with the information you hold on your V5 form (logbook).
Log onto the website and it will ask you for the document number on your V5 form.
Edit - No, it is either the V5 form or the V11 reminder form. I enjoy done it several times without the V11 form!
